This form must be filled out to reserve the church facilities. Please check availability on the ChurchTrac calendar before submitting the form. All facility reservations must be made by a church member or the church member sponsoring the request. That member must be in attendance at the event and will be responsible for what happens there and that the building is properly cared for and cleaned. The form will be sent to the elders who will evaluate and respond to each request within one week of receiving it.  


*Facility should be returned to its original condition with tables and utensils cleaned and returned to their original position, floors vacuumed, garbages emptied and taken to dumpster, and bathrooms tidy.
